Hair condition questionnaire

A few questions that genuinely change how the appointment goes. What your hair has been coloured with before, and what treatments it has had, decides which products I use and how much work the planned result will take. It takes about two minutes.

Please tell me before the visit if you have ever reacted to cosmetics or hair dye, if your scalp is irritated or affected by a skin condition, if you are pregnant or breastfeeding, or if you take medication that affects your hair and skin.

I am not assessing any of this medically — I need it to choose products and to decide whether to go ahead, postpone, or run a patch test first. If anything is unclear, I will ask you to check with a doctor before the appointment.
